Steps to Prepare For Your Session

After we have scheduled a session, these 4 emails will be coming your way. Please return the 3 forms listed below:

#1: EMAIL CONFIRMATION/FORM: Once you have scheduled a session, you will receive an email confirmation from me. This email will have a Client History Form attached as a fillable PDF. Please complete and return this before our first session.

#2: AGREEMENT: An email will be sent from Square with a Client-Therapist Agreement to review, sign and return. The form contains my office policies, information on how confidentiality works and a HIPAA notice.

#3: ZOOM MEETING: You will receive a separate email from me with a Zoom meeting link. Check your email inbox for this invite each time we schedule a session.

#4: PAYMENT: The final email to look for is a Credit Card Authorization Form from Square. Once completed, this form will authorize me to process your session payments. (You will be charged no earlier than 24 hours before each session. )
